Have you tried putting these settings in a
http://httpd.apache.org/docs/mod/core.html#virtualhost
which is only valid in the server config (/etc/apache-perl/httpd.conf
or something like that) not in .htaccess-files?
I think there is support for AG working outside a virtualhost, but try
to get the basic setup described in INSTALL working first.

You can edit Gallery.pm to include output. When I have trouble with AG
I sometimes stick lines like
print STDERR "Apache::Gallery::handler set filename to '".$r->filename()."'\n";
into Gallery.pm.
You can use
# updatedb
$ locate Gallery.pm
to find Gallery.pm.
